RNScore combines ten workload ratings, weighted category bonuses, and a separate deterioration modifier before assigning a patient level from 1 to 5. It then treats those levels as assignment points and compares totals with displayed target bands: 7–10 for a 3–4 patient PCU or step-down unit and 8–12 for a 4–6 patient med-surg or lower-acuity telemetry unit.
