Each modality receives separate grades for biological mechanism and human outcome, preventing a plausible laboratory pathway from standing in for demonstrated patient benefit. The visible rubric runs from replicated randomized evidence through case reports to no human outcome data, while source records can be marked verified, pending, or retracted. Examples include tumor treating fields with A-level evidence for glioblastoma and fenbendazole with D-level evidence, alongside a warning that the site provides no dosing instructions.
