UnityForm describes a local-network setup: one small computer stays in the clinic while paired tablets collect answers, allowing check-in to continue through an internet outage. The workflow includes language selection, side-by-side original and English answers, recorded edits, and output as the clinic’s own PDF or an export file. The site lists twelve languages, but says only English and Spanish are fully translated today. It is explicitly a pilot using practice records rather than real patient information.
