Field notes
The form accepts a map point or typed address, then asks the resident to choose a category and provide details; photos are optional, with one water-report prompt recommending them. The listed categories cover water, rubbish and dumping, roads and paths, internet, and electricity, while fire reporting is marked as coming soon. The page says report details stay private to the submitter and administrators and that a reference number is issued for follow-up.
