The score is intended to be recorded serially rather than used as a one-off diagnosis, allowing a team to distinguish admission status from later deterioration, deconditioning or recovery. Its scale is divided into 21 levels, while a separate page adds social history and package-of-care information. The site explicitly limits the measure to functional mobility and says wider assessment of gait, balance, strength, cognition, equipment and rehabilitation potential remains with physiotherapy and occupational therapy.
